With your puppy you will find an index card with all the health information on this puppy since birth. Please provide this information to your vet who will then put your puppy on his/her program of care. Schedule your vet appointment for no later than 3 days from the arrival of our puppy.
I, Tammy Johnson guaranty this puppy to be healthy at the time of purchase and delivery. Your puppy will be current on all vaccinations and worming. I will guaranty the puppy for 10 days from the date of purchase and delivery of your puppy against Parvo virus, Distemper, Leptospirosis, Hepatitis, Parainfluenza, and corona Virus. **VERY IMPORTANT** You are required to take your puppy to a qualified vet to complete the vaccine protocol for your puppy. Failure to do so, makes this warranty null and void.
I, Tammy Johnson, guaranty this puppy to be free from genetic defects that are detrimental to the puppy's health for one year from the puppy's date of birth. You are required to take this puppy to a qualified vet within 3 days of receipt of the puppy and to notify me at 417-443-0965 immediately in the event that your vet finds any problem with the puppy. I will pay for treatment for the puppy for any covered problem during the guaranty period provided that my vet agrees on the diagnosis and treatment. Any treatment for the puppy must be authorized by myself and my vet in advance and I will cover such treatment up to the purchase price of the puppy. I will not pay for the office visit to your vet or for any testing or treatment not authorized by my vet in advance unless it is a life threatening emergency and my vet agrees that such treatment was necessary and should be the responsibility of myself. I reserve the right to determine if the puppy will be returned for refund or replaced. No other guaranty is made or implied except as stated in this written guaranty
The buyer agrees that my obligation ceases and this contract is void if the buyer does not continue the vaccination series, fails to see a qualified veterinarian within three days, deprives the puppy of any medical treatment needed, or physically neglects or abuses the puppy in any way.
